2. Schedule an appointment

3. Medical History Form:
Once you schedule your first appointment, you will receive an email requesting you complete your medical history form electronically prior to your visit.

4. Past Medical Records:
If you would like us to obtain your past medical records, please submit the Record Request Form to your past provider.

5. Access your health information:
After your first visit, you will receive a link to access the Elation patient portal.
